I think the User/Registration tracker is what you want (it is what I use to collect this type of data from users during registration). Tiki won't "combine" the tracker into a single page, however — it will be 2 separate pages.
After a users completes the initial Registration page (tiki-register.php) and clicks SUBMIT, your tracker will appear, where you can collect the additional info.
